Recent presentations at ARVO and ASGCT highlighted several important developments in the treatment landscape for geographic atrophy (GA) and inherited retinal diseases, reinforcing growing industry interest in durable gene therapy and approaches that combine neuroprotection with complement pathway modulation.

1. The Field Is Moving Beyond Complement Monotherapy

Multiple real-world studies presented at ARVO highlighted the challenges and limitations associated with complement inhibitor monotherapies. Real world patient compliance is relatively poor due to the low visual benefit and risk of new-onset wet AMD. There is a growing consensus on the importance of a durable treatment and the ability to address multiple drivers of GA and retinal degeneration.

      2. Growing Validation of Neuroprotection and PEDF Biology

There was convergence of evidence supporting pigment epithelium-derived factor (PEDF) as a neuroprotective strategy for retinal disease. New basic science presentations further elucidated PEDF receptor biology and downstream signaling pathways associated with photoreceptor and retinal pigment epithelium (RPE) protection. Complementary translational studies demonstrated photoreceptor rescue and preservation across multiple disease models and delivery approaches. Importantly, clinical data presented from an investigator-sponsored study of a PEDF gene therapy demonstrated encouraging long-term visual outcomes, supporting the clinical therapeutic potential of PEDF-mediated neuroprotection in retinal degeneration.

      3. Independent Validation of Dual-Pathway Therapeutic Design

Other scientific presentations independently converged on therapeutic architectures combining multiple mechanisms of action to target complex disease pathologies including addition neurotrophic or neuroprotective factors to treat GA. These programs provide additional validation for the broader scientific rationale that addressing both inflammatory and degenerative pathways may be necessary to achieve meaningful clinical benefit in retinal disease.

      4. Benefits of Gene Therapy Durability

Gene therapy offers a durable and steady approach compared to frequent eye injections that cause drug levels to rise and fall over time. These fluctuations, along with the challenge of keeping up with repeated treatments, can reduce real-world effectiveness. In eyes with an inherited retinal dystrophy, it was shown that a single subretinal dose of an approved AAV gene therapy improved visual function over 5 years. Another Phase 2 study found that patients receiving an anti-VEGF subretinal gene therapy had a more stable retinal thickness that tended to achieve better vision outcomes while requiring fewer supplemental injections.

As a result, the value of gene therapy may extend beyond reducing treatment burden. By delivering a steady, long-lasting therapeutic effect, it may also help improve visual outcomes compared with treatments that require frequent repeat dosing.

      5. Ikarovec’s IKAR-001 gene therapy for Geographic Atrophy

IKAR-001 brings together all four key themes. It is a one-time subretinal gene therapy for geographic atrophy that goes beyond complement inhibition alone. The therapy combines PEDF-driven neuroprotection with complement regulation in a single construct, and is designed to preserve visual function, which current approved therapies do not. This dual approach reflects the direction the field is increasingly taking. By using sustained gene expression, IKAR-001 is designed to provide long-lasting, steady treatment without the fixed, frequent injections that current approved therapies require. In many ways, IKAR-001 was built around the same principles now being highlighted by ARVO and ASGCT.

Novel capsid technology to enable convenient delivery in the doctor’s office of IKAR-003 for intermediate AMD, designed to prevent progression to sight-threatening disease

NORWICH, UK & CHICAGO, US, January 6 2026 – Ikarovec Ltd, which is developing dual-pathway gene therapies for vision-threatening retinal diseases, and VectorBuilder, a global leader in gene delivery technologies, have entered into an exclusive worldwide option agreement for VectorBuilder’s novel AAV capsid technology to be used in combination with Ikarovec’s gene therapy candidate IKAR-003 for intermediate age-related macular degeneration (AMD). Following successful further evaluation of the technology, the companies will engage in a strategic partnership under which Ikarovec will be responsible for clinical development and commercialisation of IKAR-003. Based on the potential of IKAR-003 in intermediate AMD, the proposed deal is expected to be worth in excess of $1bn.

VectorBuilder’s proprietary capsid technology will allow IKAR-003 to be delivered to the eye via the minimally invasive intravitreal route in a doctor’s office. This will enhance access to the treatment for the target patient population – those at the earlier, intermediate AMD stage for whom prevention of disease progression is a priority. IKAR-003 is designed to prevent this progression through a one-time injection of an AAV-delivered dual-pathway gene therapy combining neuroprotection and complement modulation to preserve visual function.

Patients with intermediate AMD, a condition which affects millions worldwide and currently has no approved drug treatments, are at significant risk of progressing to geographic atrophy or wet AMD, both of which cause irreversible vision loss.

‘Partnering with VectorBuilder on IKAR-003 increases the value of Ikarovec’s pipeline by creating an office-based intravitreally delivered product, ideal for large-scale adoption in the prevention of disease progression setting.’ said Thomas Ciulla, MD, MBA, President and CEO of Ikarovec. ‘We have assessed other intravitreal capsid options and are confident that VectorBuilder’s technology will provide superior efficacy.’

‘IKAR-003 uses the same dual-pathway approach as our lead programme, IKAR-001, which remains on track for clinical trial initiation in late 2026 in geographic atrophy, using validated subretinal delivery in this advanced stage of disease. We are therefore addressing two distinct patient populations with a delivery approach tailored to each.’ he added.

‘We have worked with Ikarovec for several years and have developed a deep understanding of the company’s approach and technical requirements’. said Dr. Bruce Lahn, Founder and Chief Scientist at VectorBuilder. ‘By utilizing our AI-powered DeepCap platform that integrates rational design with ultradeep search of AAV sequence space, we were able to engineer best-in-class ocular capsids. In non-human primate studies, our intravitreally administered capsids can target wide areas of the retina and successfully transduce virtually all cells of the macula, demonstrating broader and more robust transduction than current clinical intravitreal capsids. We are confident that our capsid technology, combined with Ikarovec’s innovative dual-pathway therapeutic approach and experienced leadership team, will create a strong foundation for developing transformative therapies for patients with vision-threatening retinal diseases.’

Norwich, UK, May 28 2025 – Ikarovec, a leader in ophthalmology innovation with a sophisticated field-changing technology for the long-term relief of prevalent eye diseases, has highlighted the therapeutic potential of its approach in geographic atrophy and wet AMD, with presentations at major ophthalmic and gene therapy conferences. The new preclinical data discussed at ARVO1 and ASGCT2 show how the company’s flexible bicistronic gene therapies can generate multiple therapeutic proteins where they are needed in the eye, protecting retinal cells and preventing damaging blood vessel growth and scarring.

The lead candidate IKAR-001 will proceed to a geographic atrophy clinical trial supported by studies showing preservation of cells critical for vision. The preclinical studies presented on the investigational agent, which encodes the therapeutically relevant proteins pigment epithelium-derived factor (PEDF) and soluble CD46, demonstrated its potential to improve clinical outcomes in geographic atrophy, an advanced common form of AMD.

Additional studies presented at the conferences showed how another of the company’s bicistronic gene therapies reduced damaging new blood vessel formation, vascular leakage and scarring in preclinical models of wet AMD. Encoding inhibitors of vascular endothelial growth factor (VEGF) and connective tissue growth factor (CTGF), two proteins involved in driving disease progression and scarring respectively, this dual-target approach could enhance outcomes in wet AMD sufferers.

Dr Thomas Ciulla, President & CEO of Ikarovec, said, “Sufferers of devastating eye diseases such as geographic atrophy and wet AMD are in real need of effective disease modifying treatments. The new preclinical data we have presented shows the potential of Ikarovec’s proprietary bicistronic gene therapy approach to generate these improved outcomes for patients, producing multiple therapeutic proteins in the eye to protect cells and reduce damage.”

“In addition, our approach aims to dramatically reduce treatment burden on patients, by replacing frequent, often monthly, eye injections with a single administration, potentially improving both compliance and long-term outcomes,” he added. “Encouraged by the evidence generated to date and supported by our recent fundraising, we are working to complete advanced preclinical studies with our lead candidate IKAR-001, ready for the milestone of a clinical trial.”
